before i got my '03 Xtreme i owned a 1989 GMC S15 with the 4.3... and i had the EXACT same problem... what my problem turned out to be was the EGR valve, on my truck i simply unplugged the vaccum line from the EGR valve itself (making sure of course to plug the vac line as to not have a nice lil vaccum leak lol) and it never hesitated on startup again. Oh and how i figured it out... i got the truck cranked.. mind you it was hesitating, spitting and wanting to cut off... i simply reached back and pulled that vac line off of the EGR valve and it instantly smoothed out and idled normally.

and is the fuel filter on the inside of the chasis on an 85 or in the engine bay?

Mine is doing the same thing. It is a 2001 LS 4.3L 4x4. I too just had some maintenance done and a few days later it started acting like this. The work that was done is new fuel filter, oil change and filter, tranny flush and filter, and I think that's it. What could the problem be? Almost acts like timing is off. It has full power, but when it starts to fire, the motor lags really bad and spits and sputters and power dims. Someone said it could be EGR? Where is it located on my truck and what does it look like? Thanks.

Give people time to post. Its the hoolidays and many members are travelling or spending QT with family.
One thing I would check into is the battery. Sometimes the cable ends build up resistance and causes low amp and low voltage to flow. Ive seen that in reference to the lack of RPMs allowed to produce since the coil has little input volts. Use a multimeter and test the batt unhooked from the truck. Then put the terminals back on and test down the cable past the terminal (there is a free spot on the front of the Fuse/Relay Center on 98-up models where the batt cable connects).
A couple other thoughts, Fuel Pressure and Spark Module

battery is good -- it's brand new and I had the electrical system checked out. I am thinking it is starter solenoid. It turns over perfect, but right when it seems like it is going to start, it just cuts out and acts like there is a major draw of energy. It is at stealership getting diagnosed right now.

New rotor and cap fixed the problem.
